Reduce Operational Costs to Alleviate the Burden of Economy & Health Care Reform

Continuing economic pressures and recently enacted Health Care Reform mandates place additional pressure on all health plans and managed care/payers to improve their operational efficiencies and reduce associated costs. Future reductions in Medicare reimbursement rates and increased competition will also require everyone to “do more with less money.”  Industry mandated changes for HIPAA X12 5010 transactions and ICD-10 Diagnosis codes will also add to these efficiency pressures and stretch current limited project staff resources even more.

The new ICD-10 code changes will have a direct and significant financial business impact on revenues on health plans, payer organizations and health care providers. The ICD-10 code set change is not just another new IT project. The ICD-10 changes are much more than simple cross references from the old ICD-9 codes to the new ICD-10 codes. In fact, these types of simple cross walks will simply not work. For many of the new ICD-10 codes, there is a “one to many” code range from the old codes to the new codes. There will also be a significant internal staff retraining effort required with this new code set. This will not be accomplished by simply providing all staff members with a simple cross reference list. For example, some of the areas that will be directly impacted are: provider contracts, benefits plans, performance related reporting (both internal and external), and all related risk severity adjustments to name a few.

Now is the time to start educating and preparing each organization’s key stake holders on the specific changes required in implementing the new ICD-10 code sets, the required resources to implement them, and the required budget allocations to effectively accomplish the new mandated changes. ICD-10 codes must be implemented in the next couple of years. They are not going away!

My Journey to becoming a Managed Care Consultant

After graduating from college, I found myself thrown into the great unknown without a clue as to what I wanted to do for the rest of my life. Through an internship, I met a man who was married to the supervisor of a health network; her department had an opening for an Eligibility Analyst and I quickly made my entry into the health care world. I must admit that the Health Insurance industry found me, not the other way around.

Back in those days, doing eligibility meant that I would get printouts of members who were active that month and would sit for hours making tick marks on who was eligible and who wasn’t. I would then term those not on the list, add those who were, and make changes to those who needed updating. After a couple years of this (and rapidly diminishing eyesight), I was able to load eligibility electronically, which saved time so I could help out in other areas of the business as well.

As the claims volume grew, so did the need for a system that could process claims more efficiently. After the new claims system was purchased, I was chosen to be on the team involved in the conversion. During the kickoff meeting, I met four consultants who were hired to analyze our business process and advise the best approach to set up the new system. They knew everything about the system and were invaluable to the implementation. Those four consultants changed the course of my career, as I realized that I wanted to be a consultant so I could make a difference, too.

After over 12 years of Eligibility, Provider Maintenance and Provider Contracting, I took a leap of faith and joined HCIM as a Client Services Consultant. Now, I implement and configure claims management systems in addition to working on benefits, contracts, Providers, enrollment, fee schedules, and anything else that the client needs assistance with. Recently, a client was experiencing an abnormal amount of claims pending in the system, so I helped to determine the problem and recommended how to resolve the issue. I’m currently working on a project where the client is bringing up a new line of business.

I’ll be celebrating two years with HCIM this month and I must say that I couldn’t have found a better company to work for and I truly love what I do. I’ve found a career that challenges me and allows me to help people with the same frustrations that I used to have. I’ve come a long way and I have HCIM and those four consultants to thank for it.

Successful Joint ikaSystems Implementation/MC400 History Conversion Project

HCIM and IBA Healthcare Consultants, Inc. teamed up to bring a new client live with ikaSystems in only 60 days, which included converting the client’s historical data from a legacy MC400 system. This joint implementation effort successfully created over 1,300 benefit plans, migrated over 2,000 providers along with their related contracts, and migrated multiple years of historical eligibility, claims, and authorizations into the client’s new ikaClaims system.

“One of the key factors to our success stems from our deep systems and extensive operational managed care experience. We were able to quickly understand the client’s business requirements and architect ikaSystems to support current operations, integrate historical data structures, and support the client’s long term growth plans as well,” said David Lontok, President of IBA Healthcare Consultants, Inc., an HCIM business affiliate.

Don’t miss out on Business Process Automation Opportunities

I recently visited one of our current SymKey® for EZ-CAP® clients in the Midwest. We met with our client’s COO for a couple of hours on a Monday morning. She shared with us the exciting activities and long term strategies that their organization has been working on in their efforts of continuous improvement and striving to bring ongoing “value-add” to the physician groups that they manage. As one of their strategic partners, she asked us to work with her operations staff to identify new business process automation opportunities to increase their claims auto-adjudication and the use of our SymKey for EZ-CAP product that they have been licensing for the last 18 months. She just knew that additional opportunities existed. I told her that we are always available and willing to work with our clients on these operational efficiency improvements efforts. We will be scheduling an onsite trip to work with this client in the next one to two months. When our customers further leverage their utilization of SymKey, their return on investment is increased and operational gains are achieved, which results in real bottom line gains.

I like to refer to these efforts as “peeling back the layers of an onion.” Just as someone peels back each layer of an onion to find a new fresh layer, so must operational managers and staff continuously review and identify current manual business processes that can be automated. Historical decisions as to why certain manual processes were established must be revisited and  re-analyzed on a regular basis (every six to 12 months). The justification for the manual process must be questioned and new opportunities must be explored to automate current processes with the use of tools such as SymKey. In today’s health plan/payer operations, there are generally several layers of operational processes and pending of claims that can be reviewed and analyzed on a regular basis to identify the manual processes performed by staff that are either outdated or that can be automated.

HCIM’s support staff and operational consultants are always willing and available to work with our current and prospective clients to identify new opportunities to further utilize SymKey’s automation and consistent processing of claims and authorizations. I am looking forward to going onsite and working with this specific client as a strategic partner to assist them in peeling back new layers of the onion and further increasing the automated-adjudication of claims with SymKey for EZ-CAP!

Tips & Tricks: Reducing Workplace Headaches

Do members of your team have to print everything to avoid getting headaches from reading off their monitor? Here are a few simple ideas to reduce headaches, improve efficiency, and cut back on printing costs.

20-20-20 Rule
Every 20 minutes, look away from your screen at an object roughly 20 feet away, and keep your eyes there for at least 20 seconds. This will help to reduce the harmful effects of extended computer usage:

  • Headaches
  • Neck and upper back aches
  • Blurry vision during and after computer use
  • Eye strain, possibly causing changes in vision
  • Burning, itching and watery eyes
  • Overall loss of short-term focus until eyes adjust

Increase Font Size
Applications: In Microsoft Office applications, simply increase the Zoom from the View menu or hold “Ctrl” and scroll up or down with your mouse’s scroll wheel to zoom in/out.
Web Pages: From Internet Explorer, navigate to View>Text Size and select a larger font size or hold “Ctrl” and scroll up or down with your mouse’s scroll wheel to zoom in/out.
Windows: To change the size of the text in your various Microsoft windows, simply navigate to Control Panel>Display>Appearance and change the default font size from Normal to Large or Extra Large.
E-mail: Change the default font size in your e-mail client (method varies).

Reduce Glare
To reduce monitor glare, consider purchasing a glare filter. For more tips on reducing monitor glare, click here.

Buy a New Monitor
If the monitor is simply too small, consider purchasing a larger one, preferably a flat screen LCD, which produces less glare and is flicker-free – this will save money over time, by reducing printing costs and increasing employee efficiency (less headaches would also be a morale booster!).

During my time spent as an Operations Manager, I remember that my claims processors had sticky notes all over their monitors and cheat sheets that they would use to remind them of manual or special processes for certain providers or members. Not everyone had the same sticky notes, nor did they all have the same handwritten notes on their cheat sheets. This was always an impediment to providing consistent service and claims adjudication results. Use Draft Print Quality
Changing your printer’s default settings to Draft printing quality can also save a lot of ink, and you could always switch back to Normal quality for documents that require it.

Print Less

Ask an Expert Blog Launch

This month, HCIM is launching its new Ask an Expert discussion blog. The intent is to create an interactive discussion blog that leverages the hands-on experience and managed care/health plan payer expertise of HCIM’s colleagues, consultants, business affiliates, and industry leading clients. The wealth of knowledge within this expansive group provides a forum to access the best practices and answers to common issues and challenges that each of us in our collective industry encounter on a regular basis. I had been looking for a forum that I could tap into that would provide me with the opportunity to share and learn from others’ prior experiences, so I would not have to waste time “re-inventing” the wheel. I have not found such a forum to date, so I and others within HCIM decided to create one. This new Ask an Expert discussion blog is an opportunity for all of us to share our past successes and trials by contributing in an open and meaningful forum with our peers, partners, and customers.

During the month of April, HCIM is offering a $5 Amazon.com gift card for each question that is submitted, approved, and posted to the new Ask an Expert blog, with a limit of four gift cards per person. Industry Trend to Reduce Operating Costs and Gain Operational Efficiencies

During the first part of this new year, we have experienced increased inquiries and requests for product demos of our SymKey® product solution. In fact, we licensed one new SymKey® for EZ-CAP® client last month and are currently working on licensing arrangements with two prospective new clients. I believe this increased interest is due to SymKey’s ability to deliver immediate results and significant bottom line Return On Investment (ROI) in automating manual claims processing and auto-adjudication of claims within the many managed care/health care payer core transaction systems that SymKey integrates with. There are increased demands to reduce operating costs and gain operational efficiencies while experiencing reductions in membership and/or reduced premium revenues in many segments of today’s managed care/health plan payor market. HCIM’s SymKey product and other product solutions deliver on these expectations for our clients.

Some of our new SymKey clients have made the decision to bypass our 30-day risk-free SymKey trial period and go straight to licensing it, saying the decision was a “no brainer” for them.
